Die Neandertaler gehören ebenso wie der Homo sapiens zur Gattung Homo und teilen etwa 99,7 % ihrer DNA mit dem modernen Menschen (Reynolds und Gallagher 2012). Beide Arten lebten sogar kurzzeitig zur gleichen Zeit in Eurasien. Allerdings entwickelten sich die Neandertaler in Europa getrennt vom modernen Menschen, der sich in Afrika entwickelte.

Das Skelett des Neandertalers war wesentlich robuster, was darauf hindeutet, dass es mehr Platz für Muskelansätze gab. Der Neandertaler war zwar kräftiger als der moderne Mensch, aber die durchschnittliche Körpergröße des männlichen Neandertalers war kleiner und betrug nur etwa 1,70 m.
Weitere körperliche Merkmale, die den Neandertaler vom modernen Menschen unterscheiden, sind bestimmte Schädelmerkmale. Der Schädel war im Allgemeinen niedrig und länglich und wies eine abfallende Stirn mit einem Hinterhauptbein (einem Knochenvorsprung an der Rückseite des Schädels) auf, während der moderne Mensch eine eher vertikale Stirn ohne Hinterhauptbein hat. Das Schädelvolumen des Neandertalers war mit 1.500-1.740 cm³ ebenfalls größer als das des modernen Menschen, und er hatte kein Kinn und eher runde Augenhöhlen, im Gegensatz zum Homo sapiens, der ein Kinn und eher rechteckige Augenhöhlen hat (Wolpoff 1999). Trotz dieser Unterschiede waren die Neandertaler möglicherweise erkennbar genug, um mit dem Homo sapiens zu interagieren oder sich sogar während der Tausende von Jahren, die sie gemeinsam in Europa lebten, mit ihm zu vermischen.
Die Neandertaler lebten fast 200.000 Jahre lang in Europa und Asien und gediehen in diesen Regionen, starben aber vor 40.000 bis 30.000 Jahren aus, etwa zur gleichen Zeit, als der moderne Mensch in Europa auftauchte. Dies hat zu zahlreichen Spekulationen über die Art der Interaktionen zwischen Neandertalern und Homo sapiens geführt, zumal einige Forscher glauben, dass sie über 5.000 Jahre lang miteinander interagierten, bevor die Neandertaler zu verschiedenen Zeiten in Europa ausstarben. Eine Hypothese besagt, dass der Homo sapiens den Neandertaler verdrängte und besser an die Umwelt angepasst war, und es ist natürlich möglich, wenn nicht sogar wahrscheinlich, dass diese beiden Gruppen zu Konkurrenten um Nahrung und andere Ressourcen wurden, wobei der Homo sapiens am Ende erfolgreicher war.
Wenn solche engen Interaktionen stattfanden, besteht auch die Möglichkeit, dass der relativ neue Homo sapiens aus Afrika Krankheitserreger mitbrachte, die dem Immunsystem des Neandertalers unbekannt waren. Ein jüngeres Beispiel für diese Art von Wechselwirkung ist die europäische Expansion nach Amerika, die Krankheiten wie die Pocken mit sich brachte, die die Ureinwohner Amerikas nie zuvor erlebt hatten, insbesondere Krankheiten, die durch die Domestizierung von Tieren entstanden. Es ist möglich, dass die Domestizierung des Hundes durch den Homo sapiens zur Verbreitung fremder Krankheiten unter den Neandertalern beigetragen hat.
Unabhängig davon, ob dies geschah oder nicht, ist es sehr wahrscheinlich, dass die Interaktionen zwischen den beiden Gruppen zu einem bestimmten Zeitpunkt sehr viel enger wurden. Die Neandertaler waren in der Lage, eine Vielzahl hochentwickelter Werkzeuge herzustellen und zu benutzen, Feuer zu kontrollieren, Kleidung herzustellen und zu tragen sowie Dekorationen und Ornamente zu schaffen. Es gibt sogar Hinweise darauf, dass die Neandertaler ihre Toten mit Grabbeigaben bestatteten, eine Praxis, die auch beim späteren Homo sapiens anzutreffen ist, was darauf schließen lässt, dass die beiden Arten Ideen wie Werkzeugherstellung und Rituale austauschten. Von Spanien bis Russland wurden archäologische Stätten entdeckt, die Steinwerkzeuge von Homo sapiens und Neandertalern enthalten.
